Full conference fees include: Sunday reception, lunch on Monday & Tuesday, breakfast and breaks on Monday to Wednesday; Daily registrations include the food and beverage events offered that day. OPHA MembershipOPHA members will receive a discount. Please join OPHA via the OPHA website to take advantage of this saving. Early BirdTo take advantage of the early bird reduced fees, your registration form and payment must be received by 5 p.m. on Friday, September 25, 2009. GeneralOnline registration will be accepted until 5 p.m. on Tuesday, October 27, 2009. After that date, please register on-site at the conference. On-SiteThe on-site registration desk will be open from 5 to 8 p.m. on Sunday, November 1, at 7 a.m. on Monday November 2, and 7:15 a.m. on Tuesday and Wednesday, November 3 & 4, 2009. Please note that concurrent session, poster and roundtable presenters are required to pay conference registration fees. Conference PortfolioIn honour of OPHA’s 60th Anniversary, the OPHA and Constituent Societies are pleased to offer each delegate a portfolio inclusive of writing pad and USB key. Since conference bags will not be given, please bring your favourite bag or briefcase for conference materials. PaymentFull payment must be submitted with your registration. Please ensure that the credit card information is completed in full. Payment can be made by cheque, however you still must register on-line and select the cheque payment option. Cheques are payable to the Ontario Public Health Association. If you are paying by cheque, please mail your cheque, along with the registration ID # that will be provided once your on-line registration has been completed, to : OPHA Conference Secretary Please note that registrations will not be accepted and confirmation will not be sent out until full payment has been received. Cancellation PolicyNotification of cancellation and requests for refunds can be made to OPHA by calling (416) 999-5989 or emailing cf [at] eventives.ca before Friday, October 16, 2009. No refunds will be issued for cancellations received after that date. Refunds will be subject to a $50 administration charge. Substitute delegates are permitted with advance notification. Shared RegistrationsPlease note that OPHA is unable to track or coordinate shared registrations. Delegates who wish to share are responsible for making their own arrangements. One registration package will be provided per paid registration. |
